As we work to limit the spread of COVID-19, Environmental Health & Safety requires completing the Events & Gatherings Form as the first step to organizing an event or gathering. This process will allow the EH&S Safety Team to review plans, provide guidance, and approval for the event or gathering.

Event Services will confirm that any work requests have completed this process with EH&S prior to providing any equipment or services.

Times: Enter the applicable times. If you only want the equipment delivered, enter a time in the Delivery field; otherwise use the Setup field. Start and End times are required for events.
